Transition Project @ Crossroads Care
Are you aged between 14 - 18 years and thinking about your future?
Are you the Carer of a young person with Special Educational Needs and/or Disability and thinking about their transition into adulthood?
Do you live in the London Borough of Richmond?
This space enables young people to explore their futures and assists in creating opportunities. It focuses on CV writing, support with school/college work and essential life skills in a fun, safe, social and supportive environment.
By referral only. If you would like your child to attend, please contact
Crossroads Care directly, your school SENCO, nurse or teacher.
The Transition Project runs during once a week term-time, usually on a Saturday
